*** Parents must complete the Publicity Authorization and Release forms for Virtual Culminations in order to participate.

 

To sign the culmination form, you can do the following:

  1. Come to the school office from 8-11am and sign it there.
  2. Come to the school from 8-11am, pick up the form, sign it at home and bring it back.
  3. Print the form attached to the message in Remind or ClassTag, sign it and bring it to the school office in the morning.
  4. Print the form attached to the message in Remind or ClassTag, sign it, scan it or take a picture of it and email it to your homeroom teacher. You can also send the picture of the form to your teacher in Remind or ClassTag. The picture needs to be crisp and clear. We need to see the signature at the bottom.
